The Grayson County Health Department has released its latest restaurant inspection results.

The rating score of the establishment is determined by subtracting weighted point values of noted violations from 100. Unannounced inspections observe the overall operation including food source and protection; personnel; food equipment and utensils; maintenance of water and sewage systems; garbage and refuse disposal; insect, rodent and animal control; and storage of toxic items, among others.

Critical violations, four and five points in weight, are items which if uncorrected carry the potential to be an imminent threat to the public’s health. These items must be corrected in a time designated by the health department not to exceed 10 days.

Several restaurants recorded double-digit violations, including Rally’s Hamburgers (12), Sonic (11) and Huddle House (10). On the opposite end of the spectrum, FiveStar recorded 100s on both its food and retail inspections and Grayson County Middle School and the middle school tennis and football facilities recorded perfect scores. Knights of Columbus also scored a 100.

With many poor initial scores at several establishments, there were multiple follow-up inspections conducted.

Retail violations: Handwashing facilities not supplied/accessible, OTC meds expired, bulk ice sold at store without proper label, mops not stored properly, single use/single service articles not stored properly.

Fastway/Valero #14 Caneyville: Food score 95; Retail score 97 – Food violations: Hot holding temp out of range (voluntarily discarded) food contact surface not easily cleanable. Retail violations: Product not stored properly to prevent contamination in walk in cooler, single use/single service articles not properly stored, ceiling tiles in poor repair, multiple light covers with mold like substance.

Redneck Ranch Restaurant: Food score 94 — Violations: Thermometers missing in multiple areas, in use wiping cloths not stored properly, in use utensils not properly stored, food/non-food contact surfaces not easily cleanable, fans with debris in food areas.

Matia’s Pizza and Wings: Food score 97 – Violations: Handwashing facilities not supplied, thermometers date marking on multiple coolers/freezers, in use wiping cloths not properly stored.

The 19th Hole: Food score 94 – Violations: No Person in Charge (PIC) present at time of inspection, no thermometers present in multiple freezers/refrigerators, wiping cloths not properly stored, single use/single service article not properly stored.

Ready Mart: Food score 100; Retail score 98 – Retail violations: Bulk ice sold not properly labeled, single service/ single use articles not stored properly.

Papa John’s: Food score 95 – Violations: Hand washing facilities not properly supplied and accessible, missing date marking on multiple items in refrigerator/cooler, mops not properly stored, outside opening not closed to prevent entrance of insects or rodents.

Bluegrass BBQ: Food score 96 – Violations: Thermometers missing or difficult to locate, in use wiping cloths not properly stored, in use utensils not properly stored, non-food contact surface with debris.

Huddle House: Food score 92 – Violations: No PIC present at time of inspection, hand washing facilities not adequately supplied, food products stored on floor, in-use utensils not stored properly, no test strips present, plumbing not maintained at hand sink in grill/food prep area, toilet facilities not maintained, no covered trash receptacles in women’s restroom, ceiling in poor repair, tiles missing (some with damage).

Grayson County Senior Center: Food score 96 – Violations: Hand washing facilities not supplied, contamination not prevented during storage-food product stored on floor, mops not properly stored, single use/single serve items not properly stored.

Wendy’s: Food score 93 – Violations: No PIC/food inspection manager present at time of inspection, hand wash facilities not properly supplied, thermometers not present in multiple coolers & freezers, stock (food) stored on floor in freezer, clean utensils not properly stored, debris in bottom of coolers, freezers and on sides/front of multiple equipment, back door in poor repair (can see daylight in multiple areas), toilet seat in poor repair.

Dairy Queen: Food score 98 – Violations: Mops not stored properly, sanitizer buckets too weak.

Bailey Zip Mart: Food score 100; Retail score 99 – Retail violations: Non-food contact surface with debris-fans in walk-in cooler.

Barrett’s Chevron: Food score 93; Retail score 96 – Food violations: No PIC present at time of inspection, thermometers missing or not working in multiple freezers, in use wiping cloths not properly stored, single use/single service article not properly stored, garbage area with debris scattered around area, lids open, outside opening not closed/screened to prevent entrance of insects and rodents. Retail violations: Bulk, bagged ice for sale not properly labeled, single use/single service article not properly stored, garbage area with debris scattered around area, lids open. FiveStar: Food score 100; Retail score 100.

Fastway Food Mart #4 Leitchfield: Food score 99; Retail score 97 – Food violations: Single use/single service articles not stored properly. Retail violations: OTC medication expired, single use/single service articles not stored properly.

Rally’s Hamburgers: Food score 87 – Violations: Vomit and diarrheal kit needed, improper eating, tasting, drinking or tobacco use, food adulterated (recommend discard all ice and sanitize machine before put back in service), all procedures for time as a public health control not in use, hanging thermometers needed in all coolers, refrigerator, freezers, contamination during storage (on floor), mops not properly stored, non-food contact surfaces with debris, outside opening not closed to prevent/protect entrance against entry of insects/rodents.

Kentucky Fried Chicken: Food score 93 – Violations: Mops not stored properly, in use utensil not stored properly, non-food contact surfaces not easily cleanable, non-food contact surfaces with debris (multiple surfaces), toilet facilities not properly supplied, physical facilities cracked tiles around drains.

Rusty Hive: Retail score 93 – Violations: PIC not present at time of inspection, vomit/diarrheal event kit not present, hand washing facilities not supplied, test strips needed for sanitizer.

Yum Yum: Food score 99 – Violation: Lights not properly shielded.

Dairy Queen: Food Score 98 – Violations: Mops not stored properly, sanitizer buckets too weak, free standing fans in food areas with debris, condensation in freezer (on floor and around fans).

Yamato Japanese Steakhouse: Food score 92 – Violations: PIC expired, food not stored covered, ice machine with debris (mold like substance), ice scoop not properly stored.

Sonic: Food score 92 – Violations: Multiple food items out of date, food not stored covered, food stored improperly (on floor), in use utensils not stored properly, single use/single service articles not properly stored, non-food contact surfaces with debris, condensation in freezers, bottoms and sides of equipment with debris, physical facilities (broken tile), drain not functioning properly, freezer seals not functioning properly (condensation).

Taco Bell: Food score 97 – Violations: Thermometer needed in walk-in freezer and small freezer, garbage facilities-lids open, debris in and around area, vents above prep area with debris, fans in coolers with debris.
