The Grayson County Health Department has released its April restaurant/food inspection results.
The rating score of the establishment is determined by subtracting weighted point values of noted violations from 100, officials said. Unannounced inspections observe the overall operation of the facility, including food source and protection; personnel; food equipment and utensils; maintenance of water and sewage systems; garbage and refuse disposal; insect, rodent and animal control; and storage of toxic items, among others.
Critical violations, four and five points in weight, are items, which if uncorrected, carry the potential to be an imminent threat to the public’s health. These items must be corrected in a time designated by the health department not to exceed 10 days.
Inspection results
Smitty City: Food score 95; Retail score 97 – Food violations: Proper date marking missing on multiple items, wiping cloths (in use) not properly stored, food contact surfaces not easily cleanable. Retail violations: Over the counter (OTC) medicines expired, thermometers not present in freezers in back storage area.
Rally’s Hamburgers (follow-up): Food score 97 – Violations: Contamination not prevented during storage, in use utensils not stored properly.
Bailey’s Zip Mart (follow-up): Food score 96 — Violations: Hot holding temp out of temperature, ice cream scoop (water) not working.
Royal Pizza: Food score 100
Tammy’s Playschool #2: Food score 91 – Violations: Food contact surfaces with debris, toxic substances not properly labeled/identified, non-food contact surfaces with debris, food area not protected from insects, rodents & animals (exterior door propped open), physical facilities — tile in kitchen area in poor repair.
Ralph’s Market: Food score 90; Retail score 93 – Food violations: No Person in Charge (PIC) present at time of inspection, no vomit/diarrheal kit, improper thawing methods used, hair restraints not worn, in use wiping cloths not properly stored, date marking not present on a number of items in deli and coolers. Retail violations: No vomit/diarrheal kit, milk out of date, OTC medicine out of date, food contact surfaces with debris (drink machines).
Hatfield Inn: Food score 99 – Violation: Hand washing facilities not properly stocked.
Rusty Hive Nutrition: Food score 100
Leitchfield Nutrition: Food score 99 – Violation: Single use/single service articles not stored properly.
Tammy’s Playschool: Food score 97 – Violations: Thermometers not found in all freezers and coolers, hair restraints not worn, in use utensils not properly stored.
Tar Hill Quick Sack: Food score 99; Retail score 100 – Food violation: Thermometers not present in all coolers.

Las Palmas Mexican Grill: Food score 92 – Violations: PIC not present at time of inspection, missing proper date marking on multiple items, wiping cloths not properly stored while in use, in use utensils not properly stored, daylight observed at back door while closed, broken & missing tile.
Huddle House (follow-up): Food score 90 – Violations: Food not in good condition (out of date), thermometers missing in multiple areas, contamination not prevented during storage, mops not properly stored, in use utensils not properly stored, food/non-food contact surfaces not easily cleanable, ceiling tiles in multiple areas with debris.
Days Pro Bass: Food score 86; Retail score 96 – Food violations: No PIC at time of inspection, food not stored covered, food contact surfaces not clean/sanitized, proper hot holding temperatures not observed, missing date marking on multiple items, thermometers not present in all coolers/freezers, hair restraints not present during meal prep/cooking. Retail violations: Cold holding outside temperature requirements (cool temperature adjusted), thermometers not present in all coolers/freezers.

Mi Camino Real (follow-up): Food score 94 – Violations: No certified food protection manager at time of inspection, food not stored covered, no hair restraints present in food prep/cooking area, no hand washing sign posted in bathrooms, contamination not prevented during storage floor tile in poor repair.
Bailey’s Zip Mart (follow-up): Food score 99 – Violation: Ice cream scoop (water) not working in well.
Best Donuts (follow-up): Food score 97 – Violations: No certified food protection manager on site when doing inspection, hand sink not accessible or stocked, cracked/missing/broken tile.
Tammy’s Playschool #2 (follow-up): Food score 98 – Violations: Non-food contact surfaces with debris, sides of equipment, floors, rust on vent in kitchen area-in poor repair, tile in kitchen area in poor repair.
Huddle House (follow-up): Food score 92 – Violations: Certified food protection manager not present at time of inspection, thermometers missing in multiple areas, mops not properly stored, in use utensils not properly stored, food & non-food contact surfaces not easily cleanable, ceiling tiles in multiple areas with debris.
