Asian Buffet, Mi Camino Real post poor inspection scores, with multiple, critical food safety violations. McDonald’s, Yamato outstanding scores.

The Grayson County Health Department has released its October restaurant inspection results.

The rating score of the establishment is determined by subtracting weighted point values of noted violations from 100 (one hundred). Unannounced inspections observe the overall operation including food source and protection; personnel; food equipment and utensils; maintenance of water and sewage systems; garbage and refuse disposal; insect, rodent and animal control; and storage of toxic items, among others.

Critical violations, four and five points in weight, are items which if uncorrected carry the potential to be an imminent threat to the public’s health. These items must be corrected in a time designated by the health department not to exceed 10 days.

There were several outstanding scores in October, most by school-related concessions/cafeterias, as only two restaurants posted what could be considered outstanding scores: McDonald’s (98) and Yamato Japanese Steakhouse (97). Pizza Hut (95), Rusty Hive (95) and Rally’s Hamburgers (95) all scored well.

There were also poor scores recorded, with the worst being Asian Buffet, which recorded an 80 and 23 violations, including mold in the ice machine and multiple items not at proper hot holding temperature, along with several other food critical issues.

Mi Camino Real scored an 86 and had well over a dozen violations, many directly related to food safety.

American Legion Post 81: Food score 100.

Yamato Japanese Steakhouse: Food score 97 — Violations: Stored food not covered, ice scoop not properly stored.

Grayson Co. Youth Soccer: Food score 100.

Clarkson Park Concession: Food score 100.

Kirby’s Korner: Food score 99 – Violation: PIC (person in charge) not present at time of inspection.

Sonic: Food score 94 – Violations: Food not stored covered, food stored improperly on the floor, in use utensils not stored properly, single use/single service articles not properly stored, non-food contact surfaces with debris, condensation in freezers, bottoms and sides of equipment with debris, physical facilities (broken tile), drain not functioning properly, freezer seals not working properly.

Rusty Hive: Food score 95 – Violations: PIC not present at time of inspection, hand washing facilities not supplied, test strips needed for sanitizer.

Leitchfield Nutrition: Food score 94 – Violations: PIC not present at time of inspection, wiping cloths not properly stored, single use/single service items not properly stored, food & non-food contact surface not easily cleanable.

Caneyville Elementary: Food score 99 – Violation: Food stored on floor in freezer.

Grayson County Baseball Boosters: Food score 100.

Grayson County High School Concession: Food score 96 – Violations: Items in refrigerator no date marking, contamination not prevented from chemicals in storage cabinet, test strips not found.

Rally’s Hamburgers: Food score 95 – Violations: Hanging thermometers needed in all coolers/refrigerators, freezers, contamination during storage (on floor), mops not properly stored, non-food contact surfaces with debris, outside opening not closed to prevent/protect against entry of insects & rodents.

Lawler Elementary: Food score 100.

Lady Cougar Softball Concession: Food score 95 – Violations: Soap supplies out in men’s bathroom, thermometer needed in freezer, hand washing sign needed at handwashing sink, screen in poor repair on door, ceiling tiles in poor repair, test strips not found.

Grayson County High School Cafeteria: Food score 99 – Violations: Wiping cloths not stored properly.

Clarkson Elementary School: Food score 97 – Violations: Ice machine with debris.

Twin Lakes Ice Creamery Mobile: Food score 99 – Violations: Handwashing facility not accessible.

Wilkey Elementary Cafeteria: Food score 100.

Hometown IGA: Food score 99; Retail score 96 – Food violations: Cracked tiles in deli area. Retail violations: Thermometers not located in walk-in freezers, food stored improperly, rusty hooks/holders for utensils, wiping cloths not stored correctly, non-contact food surfaces in coolers with debris, ceilings, walls & floors in poor repair in several areas.

Asian Buffet: Food score 80 — Violations: Food items not in good condition (voluntarily discarded), food contact surfaces not sanitized, not using approved sanitizer for cleaning, ice machine with mold like substance, can opener with debris, cutting board in poor repair, multiple items not at proper hot holding temperature (discarded buffet pans of food), sushi rice with no proper time records, PH incorrect also, missing date marking on a few items in coolers & prep area, chemical spray bottles with no label, thermometers missing in cooler, mops not properly stored, in use utensils not properly stored, ice scoop not properly stored, scoops in products with no handles or handles down in products, utensils in use not in good repair, non-contact food surfaces with build-up & debris, inside bottom of coolers, on sides & front of equipment, physical facility-door in food area in poor repair, tiles cracked.

McDonald’s: Food score 98 – Violations: Fry freezer with duct tape on bottom of cooler, not easily cleanable surface, broken piece.

Hope Christian Academy: Food score 96 Violations: Microwave with multiple debris, single use/single service articles not stored correctly.

Mi Camino Real: Food score 86 – Violations: Food not in good condition/unadulterated (milk expired), food contact surfaces not clean and sanitized, ice machine with mold like substance/debris, clean cutting boards in poor repair/with debris, no proper date marking on multiple items in prep area & walk-in cooler, contamination not prevented during food prep & storage, food box on floor during prep, food stored on floor in walk-in, wiping cloths not stored properly (not in sanitizer buckets), mops not stored properly, single use/single service articles not properly stored, food contact surfaces in poor repair, cutting boards in poor repair, utensils in poor repair, non-food contact food surfaces with debris (bottoms of sides of coolers/freezers), physical facility tiles in poor repair in food prep area.

Pizza Hut: Food score 95 – Violations: No PIC present at time of inspection, non-food contact surfaces with debris, physical facilities (ceiling tiles in poor repair), broken tile on floor.

Little Caesars: Food score 94 – Violations: No PIC present at time of inspection, missing date marking on multiple items, single service/single use not properly stored.

Walmart: Food score 98; Retail score 99 – Food violations: Non-easily cleanable surface under hot deli case in deli area Retail violations: Meat room three compartment sink nozzle with debris, seal in ice-cream area cooler in poor repair.
