The Grayson County Health Department has released July’s restaurant/food inspection results.

The rating score of the establishment is determined by subtracting weighted point values of noted violations from 100, officials said. Unannounced inspections observe the overall operation of the facility, including food source and protection; personnel; food equipment and utensils; maintenance of water and sewage systems; garbage and refuse disposal; insect, rodent and animal control; and storage of toxic items, among others.

Critical violations, four and five points in weight, are items, which if uncorrected, carry the potential to be an imminent threat to the public’s health. These items must be corrected in a time designated by the health department not to exceed 10 days.

Inspection scores:

Farm Life Nutrition: Food score 99 – Violation: Mops not properly stored.

Mel’s Café & Catering: Food score 100.

Days Pro Bass (follow-up inspection): Food score 96 – Violations: Hand washing facilities not accessible, thermometers not provided in all coolers, contamination not prevented during food storage, single service items not properly stored.

Smitty City (follow-up): Food score 95 – Violations: Hand washing sink not accessible, thermometers not provided in all coolers, contamination not prevented during food storage, proper date marking not observed during inspection (food voluntarily discarded).

Little Caesars Pizza (follow-up): Food score 97 – Violations: Food stored not covered, non food contact surfaces with debris, physical facilities not maintained.

The Storehouse Coffee & Gifts (40 Public Square location): Food score 97 – Violations: Toxic substances not properly stored on shelving, single service articles not properly stored.

The Play Zone (follow-up): Food score 97 – Violations: Intermingling of staff & children’s food in refrigerator, three-compartment sink improperly labeled.

All Saints Episcopal: Food score 90 – Violations: No Person in Charge (PIC) present at time of inspection, food contact surface not in good repair, proper date marking not observed on multiple items, insects present at time of inspection.

Chase’s Dogs and More: Food score 97 – Violations: No vomit/diarrhea kit present at time of inspection, physical facilities with debris.

Leitchfield Sports Complex: Food score 98 – Violations: No vomit/diarrhea kit present at time of inspection.

Grayson County Senior Center: Food score 100.

Leitchfield Softball Commission: Food score 100.

Las Palmas Mexican Grill (follow-up): Food score 99 – Violation: Contamination not prevented during food storage.

Sandy’s Restaurant: Food score 95 – Violations: Time procedures not in place, in use utensils not properly stored, improper use of fly strip.

Leitchfield Aquatic Center Concession: Food score 93 – Violations: Proper hot holding temperatures not observed (voluntarily discarded food), in use utensils not properly stored, improper thawing methods, protection not present against insects, rodents & animals.

Center Court: Food score 97; Retail score 97 – Food and Retail violations: Toxic substances not properly stored, garbage not properly disposed of.

Mr. Gatti’s: Food score 100.

Pizza Hut: Food score 99 – Violation: Physical facilities not maintained.

Subway: Food score 96 – Violations: Food contact surfaces with debris, non-food contact surfaces with debris.

Smitty City (second follow-up): Food score 95 – Violations: Hand washing sink not accessible, thermometers not provided and accurate, contamination not prevented during food storage, proper date marking not observed at time of inspection.

Papa John’s Pizza: Food score 92 — Violations: No PIC present at time of inspection, hand washing facilities not properly stocked, proper cold holding temperatures not observed at time of inspection, wiping cloths not properly stored.

Huddle House: Food score 87 – Violations: Proper eating, tasting, drinking and tobacco use not observed during inspection, food stored not covered, food contact surfaces with debris, proper cold holding temps not observed at time of inspection, non-food contact surfaces with debris, physical facilities with debris, insects present at time of inspection.

Southern Specialties: Food score 91 – Violations: Food contact surfaces with debris, proper cold holding temps not observed at time of inspection, proper date marking not observed at time of inspection, physical facilities not maintained.

Walmart: Food score 95; Retail score 94 – Food violations: Food contact surfaces with debris, insects present. Retail violations: Food contact surfaces with debris, insects present, thermometers in multiple coolers in poor repair/broken.

American Legion Post 81: Food score 100.

The Storehouse Coffee & Gifts (follow-up, 40 Public Square location): Food score 99 – Violation: Single service articles not properly stored.

Leitchfield Aquatic Center (follow-up): Food score 98 – Violations: In use utensils not properly stored, improper thawing methods.

Sandy’s Restaurant (follow-up): Food score 97 – Violations: Time procedures not in place, in use utensils not properly stored.

Subway (follow-up): Food score 99 – Violation: Non-food contact surfaces with debris.

Wendy’s: Food score 88 – Violations: Food stored not covered, proper cold holding temperatures not observed during inspection, proper hot holding temperatures not observed during inspection, toxic substances not properly stored, contamination not prevented during food storage, non-food contact surfaces with debris, physical facilities not maintained.
