
The Grayson County Healthcare Foundation (GCHF) has awarded a nearly $95,000 grant to the Grayson County Health Department (GCHD) to establish a Community Health Worker (CHW) program.
The GCHF/GCHD partnership is intended to strengthen the community’s health and expand essential services to local residents.
The $94,210 grant “will allow the health department to hire and train CHWs who will serve as trusted connectors between residents and the resources they need to live healthier, more stable lives,” according to Brittany Clemons, Executive Director at Grayson County Healthcare Foundation. “CHWs will work directly with individuals and families to identify barriers, navigate available services, and link them to programs that support physical, mental and social wellbeing.”
As part of the initiative, GCHD will offer the Kentucky Prescription Assistance Program (KPAP). Through KPAP, according to Clemons, CHWs will help residents identify potential prescription savings, ensuring that cost is not a barrier to accessing necessary medications.
“We are grateful for the foundation’s support and excited to bring this program to our community,” Josh Horton, GCHD Director, said. “Our CHWs will be out in the field, meeting people where they are, and helping them connect to the services that can make a real difference in their daily lives.”
The CHW program is expected to be available by fall 2026.
“This program aligns perfectly with our mission to expand healthcare access and improve the overall health of Grayson County,” Clemons said. “CHWs are a proven, effective way to reach residents who may otherwise fall through the cracks, and we are proud to support this important step forward.”
