
The Grayson County Health Center has released its September restaurant inspection results.
Several establishments received a 100 on the inspection, including My Mom’s Ice Cream Shop, Ja Ja’s Cupcake Bakery and Catering, Hardee’s, and Grayson County High School. Posting low scores were Asian Buffet (86) and Pizza Hut (90).
Establishments begin each inspection with a score of 100. Points are then deducted for violations observed by the inspector. A score of 85 points or lower, or if the inspector observes critical violations, requires a follow-up inspection within 30 days.

Grayson County Youth Soccer – Food score 100.
Breck/Grayson Head Start – Food score 99: Violation – Electrical outlet needs repair by three compartment sink.
Rally’s Hamburgers – Food score 96: Violations – Bulk containers not properly labeled, ice scoops not properly stored between uses, walk-in cooler entrance needs repair (water leak).
Kirby’s Korner – Food score 100, Retail score 97: Violations – Outer opening not properly protected in storage area, floor tile needs repair/replaced.
Bailey’s Zip Mart – Food score 98, Retail score 95: Food violations – Thermometer not conspicuous in warm and cold holding units. Retail violations – Expired medication, expired milk product (immediately corrected), ceiling tiles need repair/replaced in men’s restroom. Follow-up retail score 99.
Sonic – Food score 94: Violations – Bulk containers not properly labeled, cooling unit door needs repair, food/non-food contact surfaces need cleaning, plumbing not properly maintained at handwashing sink, floors need cleaning, ceiling vents need cleaning, ceiling tiles need repair/replaced, walls need cleaning.
Ja Ja’s Cupcake Bakery and Catering – Food score 100.
Millwood Community Center – Food score 97: Violations – Need thermometers in all cooling units, wall needs repair in men’s room.
First Baptist Weekday Ed. – Food score 100.
Clarkson Park Concession – Food score 100.
Inside Out Grill – Food score 97: Violations – Thermometers not conspicuous in cold holding unit, cabinet doors need repair.
Kentucky Fried Chicken – Food score 92: Violations – Bulk containers not properly labeled, food not properly covered in walk-in unit, ice scoop not properly covered between uses, food/non-food contact surfaces need cleaning, floor tile needs cleaning and repair, wall needs repair by walk-in cooling unit.
Pizza Hut – Food score 90: Violations – Thermometers not conspicuous in cold holding units, warming unit has broken seal on door, countertop needs repair, food/non-food contact surfaces need cleaning, three compartment sink has faucet that needs repair, outer opening not properly protected at entrance, floor tile needs repair and cleaning, ceiling vents need cleaning, lighting not properly shielded over cooking oven.
Hometown IGA – Food score 99, Retail score 94: Food violation – Walls need repair in food prep area. Retail violations – Expired medication (immediately corrected), wiping cloth not properly stored in sanitizer in meat packing department, ceiling tiles need repair/replaced. Follow-up retail score 98.
Hardee’s – Food score 100.
Taco Bell – Food score 97: Violations – Food/non-food contact surfaces need cleaning (drink machines, grills, etc), dry storage area needs cleaning, ceiling vents need cleaning/repair.
Lawler Elementary Cafeteria – Food score 100.
Falls of Rough Resort – Food score 100.
Asian Buffet – Food score 86: Violations – Dented can food product (immediately corrected), bulk containers not properly labeled, thermometers not conspicuous in cooling units, food product not covered in walk-in unit, ice scoop not properly stored between uses, food surfaces need cleaning, hand washing sink needs repair, floor tile needs repair/replaced. FiveStar – Food score 97, Retail score 100: Violations – Sanitizer reading low, cutting board needs to be replaced.
Pine Knob Diner – Food score 100.
Leitchfield United Methodist Church – Food score 97: Violations – Thermometers not conspicuous in cold holding units, vents need cleaning in restroom facilities, lighting not properly shielded in food prep area.
Wilkey Elementary Cafeteria – Food score 99: Violation – Ceiling tiles need repair/replaced.
Grayson County Baseball Boosters – Food score 93: Violations – Thermometers not conspicuous in cold holding units, food surfaces need cleaning, possible rodent issue will require 10-day follow-up.
McDonald’s – Food score 97: Violations – Food/non-food contact surfaces need repair (seats and drink machine), food surfaces need cleaning, equipment needs cleaning throughout kitchen, lighting not properly shielded in kitchen area.
Grayson County High School – Food score 100.
Lady Cougar Softball Concessions – Food score 91: Violations – Thermometers not conspicuous in cold holding units, food/drink product stored on floor, refrigerator needs cleaning, screen door needs repair, ceiling tiles need repair/replaced.
Grayson County Middle School Cafeteria – Food score 100.
