
The Grayson County Health Department has released December’s restaurant/food inspection results.
The rating score of the establishment is determined by subtracting weighted point values of noted violations from 100, officials said. Unannounced inspections observe the overall operation of the facility, including food source and protection; personnel; food equipment and utensils; maintenance of water and sewage systems; garbage and refuse disposal; insect, rodent and animal control; and storage of toxic items, among others.
Critical violations, four and five points in weight, are items, which if uncorrected, carry the potential to be an imminent threat to the public’s health. These items must be corrected in a time designated by the health department not to exceed 10 days.
Inspection results
The Play Zone (follow-up): Food score 97 – Violations: No Person in Charge (PIC) present at time of inspection, no hair restraints worn at time of inspection, wiping cloths not properly stored.
Rough River State Park Restaurant: Food score 95 — Violations: Food not stored covered, food not properly labeled, contamination not prevented during storage, utensils not properly stored, non-food contact surfaces with debris.
Little Caesars (follow-up): Food score 93 — Violations: Hand washing facilities not accessible, food not stored covered in walk in, time as a public health control not followed, contamination not prevented during storage, hair restraints needed, wiping cloths/mops not properly stored, non-food contact surfaces with build-up & debris, floors, walls, ceilings with buildup and debris.
Yamato Japanese Steakhouse (follow-up): Food score 95 – Violations: Food stored not covered, proper cold holding temperatures not observed during inspection, in use utensils not properly stored.
Grayson Nursing & Rehabilitation: Food score 91 – Violations: Food contact surfaces with debris, date marking missing, wiping cloths, blankets & towels not used properly, non-food contact surfaces with debris, plumbing not maintained, toilet facility’s towels not individually dispensed.
Spring View Nursing & Rehab (follow-up): Food score 97 – Violations: Hair restraints not worn, mops not properly stored, physical facility (cracked ceiling tile), ceiling in multiple areas in poor repair.
Decker Boyz Raceway: Food score 100.
Yamato Japanese Steakhouse (follow-up): Food score 98 – Violations: Food stored not covered, in use utensils not properly stored.
Asian Buffet (follow-up): Food Score 92 — Violations: Hand washing sink with debris, food stored not covered, approved thawing methods not used, utensils not properly stored, sanitizer not at proper concentration, non-food contact surfaces with debris, physical facilities with debris and not in good repair.
Speedy Mart (follow-up): Food score 93; Retail score 94 – Food violations: Food stored not covered, multiple items missing dates, wiping cloths not properly stored, utensils and equipment not in good condition, non-food contact surfaces with debris, toilet facilities not open. Retail violations: Food stored not covered, multiple items missing dates, wiping cloths not properly stored, utensils and equipment not in good condition, non-food contact surfaces with debris, toilet facilities not open.
Short Creek Store (follow-up): Food score 89; Retail score 95 – Food violations: No PIC present at time of inspection, food stored not covered, proper hot holding temperatures not observed during inspection, proper date marking not observed on multiple items, food not properly labeled, toilet facilities not properly supplied.
