The Grayson County Health Department has released January’s restaurant/food inspection results.

The rating score of the establishment is determined by subtracting weighted point values of noted violations from 100, officials said. Unannounced inspections observe the overall operation of the facility, including food source and protection; personnel; food equipment and utensils; maintenance of water and sewage systems; garbage and refuse disposal; insect, rodent and animal control; and storage of toxic items, among others.

Critical violations, four and five points in weight, are items, which if uncorrected, carry the potential to be an imminent threat to the public’s health. These items must be corrected in a time designated by the health department not to exceed 10 days.

Inspection results

Captain D’s: Food score 95 – Violations: Thermometers not present in all freezers, refrigerators and coolers, contamination not prevented during storage, in use utensils not properly stored, non-food contact surface with debris, food stored without covers.

Dairy Queen: Food score 96 – Violations: Thermometers not provided in all freezers, refrigerators, mops not properly stored, non-food contact surfaces with debris, garbage/refuse not properly disposed.

Beasley’s Parkway Restaurant: Food score 83 – Violations: Hand washing facilities not accessible, food not stored covered, food contact surfaces with debris, proper re-heating procedures for hot holding not observed, proper cold holding not observed, proper hot holding temperatures not observed (Immediate correction), proper date marking not observed on multiple items, wiping clothes not properly stored/used, mops not properly stored, non-food contact with debris.

Matia’s Pizza and Wings: Food score 98 – Violations: Wiping cloths not stored properly, utensils not stored properly.

Papa John’s (follow-up inspection): Food score: 95 – Violations: No Person in Charge (PIC) at time of inspection, hand washing facilities not properly stocked, wiping cloths not properly stored.

Center Court Food Mart (follow-up): Food score 99; Retail score 99 – Food violation: Garbage/refuse not properly disposed of. Retail violation: Garbage/refuse not properly disposed of.

All-Saints Dining Hall: Food score 99 – Violation: No certified food protection manager at time of inspection.

Smitty City (follow-up): Food score 97 – Violations: Hand washing sink not accessible, thermometers not provided and accurate, contamination not prevented during food storage.

Huddle House (follow-up): Food score 94 – Violations: Food stored not covered, proper cold holding temperatures not observed at time of inspection, non-food contact surfaces with debris, physical facilities with debris.

Farm Life Nutrition: Food score 100.

Bluegrass BBQ Food Truck: Food score 97 – Violations: Food not in good condition, safe & unadulterated (voluntarily discarded), wiping cloths not properly stored.

Walmart (follow-up): Food score 97; Retail score 96 – Food violations: Food contact surfaces with debris. Retail violations: Food contact surfaces with debris, thermometers in multiple coolers in poor repair/broken.

Lady Cougars Softball: Food score 99 – Violation: Thermometers not in all coolers.
